Live consultation transcription (MERaLiON) + handover docs
Adds live consultation transcription (MERaLiON STT) wired into the Appointment view, plus end-of-internship handover docs, .env.example template, and a CLAUDE.md setup directive. No secrets committed; real .env stays gitignored. BEDROCK_BEARER_TOKEN must be swapped to the team's own AWS token; MERALION_API_KEY (shared) passed out-of-band.
